چکیده
Background Some patients with COVID-19 develop life-threatening thrombotic complications including myocardial infarction, deep vein thrombosis, pulmonary embolism, disseminated intravascular coagulation, and ischemic stroke. These inflammatory hypercoagulable states have been well documented in patient cohorts from metropolitan areas, but not more rural populations, nor has a data-driven treatment plan developed for patients. Methods We undertook retrospective case-control study of COVID-19-positive to analyze the impact thrombosis on various clinical endpoints terminal diagnosis disease severity. Prevalence were determined using medical records 2237 hospitalized Cumberland County, North Carolina. Odds ratios diagnosis, death, ICU admission, ventilation calculated based status, D-dimer level, or type antithrombotic treatment. Results Terminal [OR 1.81; 95% CI (1.10, 2.98)], admission [2.25; (1.33, 3.81)], [2.46; (1.45, 4.18)] all likely (N = 97) compared nonthrombotic 2140) after adjusting age. levels associated death overall, among Treatments that combined antiplatelet anticoagulant drugs appeared be efficacious than anticoagulants alone preventing severe disease. Limitations Patient history prior hospitalization was evaluated. Conclusion In this cohort, those are at increased risk adverse outcomes Antithrombotic therapy includes provides improved outcomes. Higher-powered prospective trials will necessary confirm any potential merits therapy.
